There have been some changes to our Conflict of Interest disclosure processes and template. Please see below for the changes that have been implemented and are in effect at this time.



1.      Our Conflict of Interest disclosure process, as well as the form, has changed. There are two different processes for completing conflict of interest disclosures, contingent upon funding.



a.      Externally funded research:  All study personnel on externally funded research must have a current, electronic financial conflict of interest disclosure complete. When receiving external funding, electronic conflict of interest disclosures are due annually and are completed at https://coi.utoledo.edu/.  You will use your utad credentials to login. Electronic financial conflict of interest disclosures are not managed by our staff however, should you have any questions you can send an email to coi.disclosure at utoledo.edu<mailto:coi.disclosure at utoledo.edu>.

Login to create new or update existing disclosures. Enter a valid UTAD username and password. All people supported on a grant or a Sponsored Program agreement or who have submitted a research proposal are required to file financial disclosures in compliance with University of Toledo polices and federal regulations.



b.      Internally sponsored, unfunded, or funded studies in which an individual does not have a Rocket number (e.g. non-UT researchers): All study personnel must complete the new COI, fillable template (see attached). It must be completed electronically and it must be digitally signed. Written responses and a physical signature cannot be accepted and the old template can no longer be accepted. Links to the COI form are provided in IRB Manager or on our website at http://www.utoledo.edu/research/rsp/irb/. Refer to the instructions on page one of the template to create a digital ID. Previously COI forms were project specific but, this is no longer the case. This document is valid for one year. As such, you are encouraged to save a copy of your COI form to be used for any projects you may be engaged in throughout the year. Be sure to complete a new form annually.
