[Irb] Updates and Reminders re: IRB reliance

In accordance with our recently updated IRB Reciprocal Agreement with ProMedica, please note that all studies that involve UToledo study personnel and that will be reviewed by the ProMedica IRB must first be submitted to the UToledo HRPP office using one of two xForms:

Upon completion, IRB Manager will generate an authorization letter to be included in your IRB application to ProMedica or another external IRB, as applicable.

These short, simple xForms do not duplicate IRB review, but they allow us to ensure all training and COI requirements are met and to notify the reviewing IRB of any relevant local context issues.

Please note that one of these two xForms must be used any time UToledo faculty, staff or students will be engaged in research reviewed by an external IRB (i.e. not only for ProMedica).
